About the Role

Vision Loss Rehabilitation Canada is seeking a highly skilled and compassionate Client Care Navigator to join our team. As a Client Care Navigator, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities
  • Coordinate and integrate supports and services around individuals living with vision loss
  • Assess client needs and develop personalized care plans
  • Strengthen communication and linkages between Vision Loss Rehabilitation Canada staff and external community healthcare providers
  • Ensure timely access to Vision Loss Rehabilitation Canada services and information
  • Facilitate linkages, communication, and information exchange opportunities between clients and service providers
Requirements
  • 3-5 years of client service experience in the health and/or social service sectors
  • Experience working directly with individuals living with vision loss
  • Knowledge of available community services and clinical, social, and residential care options
  • Strong knowledge of client-centered philosophy and clinical expertise related to client-centered interviewing, assessments, and scheduling practices
  • Excellent communication and interpersonal skills
  • Ability to prioritize workload and manage competing tasks
  • Ability to work independently and within a team
Preferred Qualifications
  • Registered health professional designation
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline
  • 3 or more years of experience working with individuals with vision loss
