Key Responsibilities- Assess client needs and develop personalized care plans to achieve optimal outcomes.
- Coordinate and facilitate linkages between clients, healthcare providers, and community services to ensure seamless care delivery.
- Provide education and support to clients and their families to empower them to make informed decisions about their care.
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
- Monitor and evaluate client progress, making adjustments to care plans as needed.
- 3-5 years of experience in client service, preferably in the healthcare or social services sector.
- Experience working with individuals with vision loss and knowledge of available community services and clinical care options.
- Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and healthcare providers.
- Ability to work independently and as part of a team, with a focus on client-centered care.
- Proficiency in technology, including electronic medical records and case management systems.
