About the Role

Vision Loss Rehabilitation Canada is seeking a skilled Client Care Navigator to join our team. As a Client Care Navigator, you will play a critical role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities
  • Assess client needs and develop personalized care plans
  • Coordinate with healthcare providers and community services to ensure seamless care delivery
  • Provide education and support to clients and their families
  • Facilitate communication and linkages between clients and service providers
  • Monitor and evaluate client outcomes and make adjustments to care plans as needed
Requirements
  • 3-5 years of experience in client service, preferably in the healthcare or social services sector
  • Experience working with individuals with vision loss and chronic health conditions
  • Strong knowledge of community services and clinical care options
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
