At Vision Loss Rehabilitation Canada, we are seeking a highly skilled and compassionate Client Care Navigator to join our team. As a Client Care Navigator, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.
Key Responsibilities:
- Assess client needs and develop personalized care plans to achieve their goals.
- Coordinate and schedule therapy appointments with primary and specialized service providers.
- Monitor client satisfaction and take remedial action as required.
- Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps.
- Ensure that all service-related activities are documented in the client health record following VLRC policies.
Requirements:
- Understanding of roles and linkages across primary care, community care, and specialized geriatric services.
- Knowledge of client-centered philosophy related to interviewing, assessments, care planning, and scheduling practices.
- Clinical expertise related to client-centered interviewing, assessments, and scheduling practices.
- Experience in client assessments and care planning/coordination.
- Excellent communication skills (verbal and written).
- Strong interpersonal skills, including shared decision-making and facilitation.
- Ability to prioritize workload, manage competing tasks, problem-solving skills, and resourcefulness.
- Proficient in technology (e.g., Microsoft Office and case management and care coordination systems).
Work Environment:
- Willingness to work flexible hours.
- Willingness to work in a variety of settings.
