Job Overview

Reporting to the Manager of Coordinated Access, the successful candidate will be an integral part of a dedicated professional team responsible for the following:

  • Collaborating closely with the Central Intake program to assess new referrals and determine the most suitable management steps.
  • Conducting standardized referral evaluations primarily through telephone communication, and occasionally via videoconference, utilizing a utilization management tool for level of care assessment in the fields of addictions and mental health.
  • Effectively matching clients with appropriate resources based on assessment results, established criteria, and clinical judgment.
  • Ensuring timely follow-up with clients and referrers to facilitate smooth navigation through the system.
  • Participating in quality improvement initiatives aimed at enhancing the services provided by the organization.
  • Performing additional responsibilities as assigned.

Required Qualifications

  • A Bachelor's degree in a social service or healthcare-related field.
  • Preferred: A regulated healthcare professional designation (e.g., Registered Nurse, Social Worker, Occupational Therapist).
  • Must be a member in good standing with the relevant regulatory body.
  • A minimum of 5 years of recent experience in delivering clinical services within the addictions and mental health sector.
  • Demonstrated knowledge and experience in clinical assessment and care plan formulation.
  • Comprehensive understanding of mental health, concurrent disorders, and addictions, including withdrawal management.
  • Strong time management abilities and meticulous attention to detail, with a proven track record of navigating complex processes and adapting to rapid changes while maintaining a client-centered approach.
  • Exceptional customer service skills, ensuring prompt responses to inquiries and appropriate follow-up to uphold exemplary patient experiences.
  • Excellent interpersonal and communication skills, with a demonstrated ability to foster positive relationships with both internal and external stakeholders.
  • Proven capability to work independently as well as collaboratively within a multidisciplinary team in a fast-paced environment.
  • Advanced proficiency in Microsoft Office applications.
  • Bilingualism (French) or proficiency in a second language is considered an asset.
  • Familiarity with the various service systems in the region is advantageous.
  • Knowledge of Meditech Expanse and/or Novari eReferral software is a plus.

Additional Information

Employment Status: Permanent Full-time

Work Schedule: Monday to Saturday, hours may vary based on operational needs.

Compensation: Competitive hourly rate based on experience.
