The Transition Coordinator - Complex (TC-Complex) position is a clinical consult-based role focused on assessing patients' needs holistically, considering their life circumstances and health determinants. This role requires strong critical thinking skills and the ability to interpret complex information from various sources, essential for making informed clinical decisions regarding suitable continuing care options.

Role Overview:

As a Registered Nurse (RN), you will deliver a comprehensive range of nursing services to patients, families, and communities, ensuring their safety and well-being. Your responsibilities will include:

  • Utilizing nursing processes through critical thinking, problem-solving, and decision-making.
  • Teaching, counseling, and advocating for patients and their families.
  • Leading an integrated and holistic approach to patient care, health promotion, and maintenance.
  • Providing safe, quality, patient-centered care that aligns with AHS's vision and values.

Important Information:

Please note that provisional permit holders will receive a specific pay rate, which will be adjusted upon registration as a Registered Nurse in accordance with AHS/UNA agreements. This position may close at a specified time but could be filled earlier if a regular employee expresses interest within a designated timeframe.

Required Qualifications:

To qualify for this role, candidates must have:

  • Completion of an accredited nursing education program.
  • Active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications:

Candidates should also possess:

  • A minimum of five years of experience as a Transition Coordinator in both acute and community settings.
  • At least two years of experience in community health.
  • Experience in home care, supportive living, or facility living.
  • Ability to work independently and collaboratively within multi-disciplinary teams.
  • Strong time management, conflict resolution, and communication skills.
  • Capability to assess clients in various environments and manage high-stress situations.

Preferred Qualifications:

Familiarity with computer applications such as Microsoft Word, Pathways, RAI Tools, Connect Care, Meditech, and e-People is considered an asset.
