Job Summary

CB Canada is seeking a highly skilled Social Worker to join our MASTRS & OASIS Programs team. As a key member of our interdisciplinary team, you will be responsible for delivering seamless services and programs to our older adult clients and customers.

Key Responsibilities
  • Conduct comprehensive assessments for clients, integrating biopsychosocial history, diagnosis, and treatment planning.
  • Develop and evaluate treatment plans in collaboration with the clinical team, community supports, and families.
  • Facilitate group counseling sessions and provide individual counseling to support patients' specific needs and goals.
  • Ensure continuity of care through the transfer of information and work with the clinical team to initiate community treatment orders.
  • Establish relationships with key community supports and develop discharge plans.
  • Prepare referrals to appropriate community services and document in a timely manner.
  • Collaborate with the multidisciplinary team to provide high-quality care.
Requirements
  • Master's Degree in Social Work (MSW) required.
  • Registered member of OCSWSSW.
  • Working knowledge of the Mental Health Act.
  • Demonstrated experience working with addictions and concurrent psychiatric disorders.
  • Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing).
  • Knowledge of standardized and non-standardized assessments.
  • Experience in program development and delivery.
  • Knowledge of local community social services.
