About the Role

We are seeking a skilled Case Manager to join our team at Persons Community Solutions LTD. As a Case Manager, you will play a critical role in supporting individuals and families in our community.

Key Responsibilities
  • Conflict Resolution: Resolve conflict situations in a fair and impartial manner, ensuring the well-being of all parties involved.
  • Coaching and Support: Provide coaching and support to individuals and families, helping them to achieve their goals and overcome challenges.
  • Compliance and Investigation: Identify and investigate compliance issues, ensuring that our organization is operating in accordance with relevant laws and regulations.

You will be working in a dynamic and fast-paced environment, supporting individuals and families in our community. As a Case Manager, you will have the opportunity to make a real difference in people's lives.

Requirements
  • Education: Bachelor's degree in a relevant field.
  • Experience: 1 year to less than 2 years of experience in a related field, or equivalent experience.
  • Computer and Technology Skills: Proficient in Google Docs and other relevant software.
  • Documentation and Reporting: Ability to create accurate and detailed reports, including accident or incident reports.
  • Screening Questions: Ability to ask and answer screening questions, including questions related to residency, experience, and certifications.
