Job Summary

We are seeking a skilled Case Manager to join our team at Pan Am Clinic. As a Case Manager, you will play a critical role in determining program eligibility and assessing the need for care at home or in a personal care home for the elderly or infirm adult.

Key Responsibilities
  1. Eligibility Assessment: Receive referrals and proceed with assessment of client and family to determine care needs related to Home Care.
  2. Care Planning: Perform assessment and prioritization of patterns/needs, establish appropriate discipline coordinator, and develop a plan of care for Continuing Care Program.
  3. Care Coordination: Implement and coordinate care plan, which may include initiating medical rehabilitation and consultative services as necessary to meet client needs.
  4. Client Support: Provide professional intervention where appropriate through professional counseling/teaching/guidance crisis intervention, etc.
  5. Case Management: Maintain current case count, ensure proper submission of statistics, and plan and organize work schedule and manage caseload demands effectively.
Requirements
  • Education: BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required.
  • Experience: Two years directly related community experience preferred. Previous experience as a Case Coordinator in Community Health Services preferred.
  • Certification/Licensure/Registration: Valid driver's license and a vehicle. Licensure/registration as per professional designation required.
Preferred Qualifications
  • Knowledge and Experience: Knowledge and experience with computers required. Effective oral and written communication skills.
