We are seeking a highly skilled Social Worker to join our team at Fraser Health. As a key member of our clinical services team, you will provide psychosocial/emotional assessment and therapeutic clinical counselling services to clients in our assigned clinical areas.
Key Responsibilities- Conduct comprehensive psychosocial/emotional assessments of clients using various modalities and in accordance with professional standards and clinical policies.
- Provide basic therapeutic clinical counselling services to clients based on a psychosocial assessment, promoting improved psychosocial/emotional wellbeing.
- Educate and provide consultation to clients and healthcare professionals on issues related to Social Work practice and area of clinical specialty.
- Maintain client records by documenting client assessments, charting client information, and preparing progress notes and reports.
- Participate in interdisciplinary team meetings and provide input to client care planning and discharge planning.
- Communicate effectively with community health and social agencies, including government Ministries, Home Health personnel, and family physicians.
- Act as a resource to clients by providing information regarding community resources and access to these resources.
- Participate in ongoing quality improvement by engaging in program/practice evaluation, identifying opportunities for change, and assessing available resources and/or gaps in service.
- Bachelor's Degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers (BCCSW).
- Knowledge and understanding of psychosocial/emotional issues related to illness/injury/disease, and current Social Work treatment modalities used to ameliorate these issues.
- Ability to communicate effectively verbally and in writing with clients, the health care team, and community agency personnel.
- Ability to work effectively with others, including working effectively in a multicultural environment, independently, as part of a multidisciplinary team, and understanding the frameworks of other disciplines.
