Job Title: Social Worker BSW, Tertiary Older Adult Program

About the Role:

We are seeking a highly skilled and compassionate Social Worker to join our team in the Tertiary Older Adult Acute program. As a Social Worker, you will play a vital role in providing comprehensive therapeutic mental health interventions and conducting geriatric assessments to support the well-being of older adult patients.

Key Responsibilities:

  • Provide comprehensive therapeutic mental health interventions to patients, focusing on their emotional, social, and cultural needs.
  • Develop and implement psycho-social care plans, including life skills training, in collaboration with patients, families, and interdisciplinary team members.
  • Assess and evaluate the need for social work interventions, planning and implementing strategies to resolve patient care problems and achieve treatment goals.
  • Provide social work counseling services, including individual, family, and resource counseling, to facilitate changes in identified non-functional behavior, attitude, feelings, and/or environment.
  • Participate in interdisciplinary activities, such as resident conferences and daily/weekly report meetings, to ensure effective communication and collaboration.
  • Maintain accurate and up-to-date patient records, documenting assessments, charting patient information, and preparing progress notes and discharge summaries.
  • Develop and deliver in-services, presentations, and training related to mental health, collaborating with interdisciplinary team members to ensure relevant content and effective delivery.

Requirements:

  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • Two years of recent related experience within mental health older adult care.
  • Current full registration with the BC College of Social Workers.
  • Current CPR Level C.
  • Valid BC Driver's License and access to a personal vehicle for business use.
