Job Summary:
We are seeking a highly skilled and compassionate Social Worker, BSW to join our team at Fraser Health. As a key member of our clinical services team, you will provide psychosocial/emotional assessment and therapeutic clinical counselling services to clients in our assigned clinical areas.
Key Responsibilities:
- Conduct psychosocial/emotional assessments of clients using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Provide basic therapeutic clinical counselling services to clients based on a psychosocial assessment.
- Develop and implement care plans that promote improved psychosocial/emotional wellbeing.
- Collaborate with clients and community service partners to ensure effective care and discharge planning.
- Communicate effectively with community health and social agencies to ensure seamless care coordination.
Qualifications:
- Bachelor's Degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers (BCCSW).
- One year recent, related clinical experience.
Skills and Abilities:
- Knowledge and understanding of psychosocial/emotional issues related to illness/injury/disease.
- Ability to demonstrate a high level of knowledge, skill, effectiveness, self-awareness, and self-correction in verbal communications.
- Knowledge of current Social Work treatment modalities and legislation applicable to social work in Health Care.
