About the Role

CarePartners is seeking a highly skilled Registered Nurse to join our team as a Home Care Specialist. As a key member of our interdisciplinary care team, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and maintain effective care plans in collaboration with patients, family members, and other healthcare professionals.
  • Provide nursing services in a variety of settings, including in-home, clinic, retirement home, and school.
  • Communicate and liaise with patients, family members, and other members of the care team to ensure seamless care coordination.
  • Work independently or in cooperation with Home and Community Care Support Services (HCCSS) to provide comprehensive care.
  • Participate in ongoing education, certification, and training to enhance your skills and knowledge.
