As a Home Care Registered Nurse with CarePartners, you will play a vital role in delivering high-quality health services to patients in their own environments. This position requires a commitment to providing compassionate care while fostering strong relationships with patients and their families.
Key Responsibilities:- Develop and implement effective Care Plans in collaboration with Community Health Services.
- Deliver exceptional nursing care to patients both in-person and through virtual platforms.
- Provide services across various settings, including homes, clinics, and schools.
- Engage with patients, families, and interdisciplinary teams to ensure comprehensive care.
- Maintain a valid driver's license and vehicle for regular travel to patient locations.
- Excellent verbal and written communication skills in English.
- Current immunization record, including a two-step TB test.
- Ability to work flexible hours, including weekends and on-call duties.
