Key Responsibilities:- Develop and implement effective Care Plans in collaboration with Community Health Services.
- Deliver exceptional nursing care to patients both in-person and through virtual platforms.
- Provide services in diverse environments, including private residences, clinics, and educational institutions.
- Engage with patients, families, and interdisciplinary teams to ensure coordinated care.
- Maintain a valid driver's license and utilize personal transportation for patient visits.
- Strong verbal and written communication skills in English.
- Current immunization records, including a two-step TB test.
- Ability to work independently and as part of a collaborative team.
