About the Role

CarePartners is seeking a skilled and compassionate Community Health Nurse - Visiting to join our team. As a key member of our community nursing services, you will provide high-quality, patient-centered care to individuals in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and implement effective care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing services, including wound care, medication management, and education, to patients in their homes and community settings.
  • Conduct thorough assessments, identify patient needs, and develop individualized care plans to promote optimal health outcomes.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure comprehensive patient care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and coordination.
Requirements
  • Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN) or Registered Practical Nurse (RPN).
  • Strong clinical skills and experience in community health nursing, with a focus on patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
  • A valid driver's license and reliable vehicle, as regular travel is required.
  • Current immunization record, including a two-step TB test, and CPR certification.
