Rehabilitation Counsellor

Aboriginal Health and Wellness Centre is seeking a skilled Rehabilitation Counsellor to join our team at the Indigenous-led RAAM Clinic. As a key member of our inter-professional team, you will provide long-term case management, systems navigation, and counselling services to individuals in need of care.
Key Responsibilities:- Provide intake and assessment, counselling, referrals, educational sessions, and development of treatment plans for individuals struggling with addiction and mental health issues.
- Collaborate with withdrawal management services, addiction treatment agencies, and other healthcare professionals to ensure comprehensive care.
- Document client information in a clear and concise manner, adhering to required formats, policies, and legal guidelines.
- Facilitate problem-solving techniques with clients, in consultation with the RAAM team and other allied health professionals.
- Support a Collaborative Care Model, promoting culturally sensitive and person-centered care.
- Completion of a recognized degree or diploma in Social Services or a related health discipline.
- Minimum six months of experience working with individuals and families managing addiction.
- Basic familiarity with addiction and mental illness presentations, as well as knowledge of relevant legislation and policies.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
