Indigenous Social Worker

About the Role:
We are seeking a highly skilled and culturally sensitive Indigenous Social Worker to join our Virtual Psychiatry Unit team at Fraser Health. As a member of our multi-disciplinary team, you will provide culturally appropriate counseling and intermediary services to First Nations, Métis, and Inuit clients.
Key Responsibilities:
- Provide social work services for clients, families, service providers, and community agencies to enhance service delivery through consultation, development of a care plan, and forwarding necessary information to appropriate community services/agencies.
- Conduct client assessments and screenings that include evaluation of emotional, physical, mental, and spiritual health as well as social determinants of health using information from all available sources.
- Participate in a multi-disciplinary team, bridging Indigenous Health and Mental Health and Substance Use, referring Indigenous clients to appropriate services, as eligible.
- Communicate with and obtain assistance from appropriate community agencies and FHA programs to ensure the best possible care for clients and families after discharge from hospital.
- Provide education and promotes health promotion initiatives/services to individuals, families, and Indigenous communities to increase knowledge, capacity, and resources to maintain holistic wellness.
Qualifications:
- Bachelor's degree in Social Work from an approved School of Social Work.
- Current full registration with the BC College of Social Workers.
- Two years' recent, related clinical social work experience in a hospital or community setting, including one year of experience working with Indigenous clients.
- Lived experience and/or knowledge of Indigenous health approaches, protocol, cultural issues, and practices.
