Indigenous Health Liaison Social Worker

Job Summary:
We are seeking a highly skilled and culturally sensitive social worker to join our team as an Indigenous Health Liaison Social Worker. In this role, you will work closely with our multi-disciplinary team to provide culturally appropriate counseling and intermediary services to First Nations, Metis and Inuit clients.
Key Responsibilities:- Provide culturally sensitive counseling and intermediary services to Indigenous clients
- Collaborate with other programs across Fraser Health and Indigenous partner organizations
- Facilitate connections between Indigenous clients and service providers
- Act as an Indigenous Health liaison for clients, families, service providers, and community agencies
- Conduct client assessments and screenings
- Participate in a multi-disciplinary team regarding Indigenous Health
- Identify clients and refer to appropriate services
- Communicate with and obtain assistance from community agencies
- Provide education and promotes health promotion initiatives/services
- Encourage informed decision-making, personal growth and learning
- Bachelor's degree in Social Work from an approved School of Social Work
- Two years' recent, related clinical social work experience in a hospital or community setting
- Knowledge of Indigenous health approaches, protocol and cultural issues
- Current full registration with the BC College of Social Workers
