Indigenous Health Liaison Social Worker

Job Summary:
The Indigenous Health Liaison Social Worker will work as a member of a multi-disciplinary team to provide culturally appropriate counselling and intermediary services to First Nations, Metis, and Inuit clients. The successful candidate will have a strong understanding of Indigenous health approaches, protocol, and cultural issues.
Key Responsibilities:
- Provide consultation to patients, families, and healthcare professionals
- Make effective referrals to community resources and services
- Collaborate with other programs across Fraser Health and Indigenous partner organizations
- Facilitate connections between Indigenous clients and service providers
- Act as a client advocate and provide education on health promotion initiatives
- Participate in multi-disciplinary teams and committees to integrate Indigenous Health with hospital and community programs
Qualifications:
- Bachelor's degree in Social Work from an approved School of Social Work
- Two years' recent, related clinical social work experience in a hospital or community setting
- Knowledge of Indigenous health approaches, protocol, and cultural issues
- Current full registration with the BC College of Social Workers
Working with Fraser Health:
The Indigenous Health Liaison Social Worker will work collaboratively with other healthcare professionals to provide high-quality care to Indigenous clients. The successful candidate will have a strong understanding of the principles, practices, and theories of Social Work/Clinical Counselling as applied to Indigenous populations.
