Indigenous Health Liaison Social Worker

Fraser Health is seeking a skilled and compassionate Indigenous Health Liaison Social Worker to join our team. As a key member of our multi-disciplinary team, you will play a vital role in providing culturally appropriate counseling and intermediary services to First Nations, Metis, and Inuit clients.
Key Responsibilities:- Provide culturally sensitive counseling and intermediary services to Indigenous clients
- Collaborate with other programs across Fraser Health and Indigenous partner organizations
- Facilitate connections between Indigenous clients and service providers to enhance access to Fraser Health services
- Act as an Indigenous Health liaison for clients, families, service providers, and community agencies
- Conduct client assessments and screenings, and develop care plans
- Communicate with and obtain assistance from community agencies to ensure the best possible care for clients and families
- Provide education and promote health promotion initiatives to individuals, families, and Indigenous communities
- Encourage informed decision-making, personal growth, and learning, empowering clients to improve their quality of life
- Bachelor's degree in Social Work from an approved School of Social Work
- Two years' recent, related clinical social work experience in a hospital or community setting, including one year of experience working with Indigenous clients
- Knowledge of Indigenous health approaches, protocol, and cultural issues
- Current full registration with the BC College of Social Workers
