Indigenous Health Liaison Social Worker

Fraser Health is seeking a skilled and compassionate Indigenous Health Liaison Social Worker to join our team.
About the RoleThis is a unique opportunity to work with a diverse team of healthcare professionals and make a meaningful difference in the lives of Indigenous clients. As an Indigenous Health Liaison Social Worker, you will provide culturally appropriate counseling and intermediary services to First Nations, Metis, and Inuit clients. You will work collaboratively with other programs across Fraser Health and Indigenous partner organizations to enhance access to healthcare services.
Key Responsibilities- Act as an Indigenous Health liaison for clients, families, service providers, and community agencies to enhance service delivery.
- Conduct client assessments and screenings to evaluate emotional, physical, social, spiritual, economic, and mental status.
- Participate in a multi-disciplinary team regarding Indigenous Health and identify clients for referral to appropriate services.
- Communicate with community agencies to ensure the best possible care for clients and families after discharge from hospital.
- Provide education and promote health promotion initiatives to increase knowledge regarding self-care and whole body health.
To be successful in this role, you will require:
- A Bachelor's degree in Social Work from an approved School of Social Work.
- Two years' recent, related clinical social work experience in a hospital or community setting, including one year of experience working with Indigenous clients.
- Knowledge of Indigenous health approaches, protocol, and cultural issues.
- Current full registration with the BC College of Social Workers.
