Position Overview

As a Registered Nurse specializing in the transition from hospital to home care, you will play a vital role in enhancing the quality of life for our clients. Your expertise will be instrumental in providing comprehensive assessments and developing tailored care plans that meet the unique needs of each individual.

Key Responsibilities

  • Conduct thorough geriatric behavioral assessments utilizing the PIECES framework to ensure holistic care.
  • Collaborate with multidisciplinary teams, including healthcare professionals and caregivers, to formulate effective behavioral care strategies during transitions.
  • Perform Best Possible Medication Histories and engage with pharmacy professionals to ensure safe medication management.
  • Facilitate communication with primary care providers and specialists to ensure continuity of care post-discharge.
  • Address client and caregiver inquiries, providing support and guidance related to the behavioral management care plan.
  • Engage in assessment, consultation, and treatment as necessary to optimize client outcomes.
  • Utilize an Assessment, Behavior, Outcomes, and Evaluation (ABOE) approach to document observations and share insights with the care team.
  • Maintain accurate records in the LHIN CHRIS database and other relevant systems to track client progress.
  • Participate in data entry to monitor program effectiveness and outcomes.
  • Act as a representative of the organization, fostering positive relationships with clients and the community.
  • Contribute to the training and orientation of new staff and students, ensuring adherence to best practices.
  • Promote a safe environment for clients and staff, adhering to established health and safety protocols.
  • Engage in quality improvement initiatives aimed at enhancing care for the geriatric population.

Qualifications

  • Registered Nurse with a Bachelor of Science in Nursing (BScN) or diploma.
  • Experience in community health care, particularly with geriatric and mental health populations.
  • Strong understanding of health care legislation and community resources.
  • Completion of a Case Management Certificate is advantageous.
  • Current CPR certification and adherence to documentation standards.
  • Experience with crisis intervention and behavioral management strategies.
  • Ability to communicate effectively with diverse populations.
