Job Summary
The Transitional Care Coordinator plays a critical role in ensuring a smooth transition of patients from hospital to home or other care settings. This position requires strong communication and coordination skills, as well as the ability to work effectively with inter-professional teams.
Key Responsibilities
- Coordinate patient transfers and discharges, ensuring timely and effective transitions
- Conduct risk assessments and develop discharge plans in collaboration with patients, families, and healthcare teams
- Identify and address barriers to discharge, including social and environmental factors
- Collaborate with Home and Community Care Support Services case managers to develop individualized plans of care
- Communicate discharge plans to patients, families, and healthcare teams
- Document assessments and discharge plans in electronic medical records
- Participate in daily bed management meetings to discuss patient discharge status and transitional requirements
Requirements
- Bachelor's degree in a health-related discipline
- Post-diploma education or experience in gerontology
- Minimum three to five years of recent acute care hospital and community experience
- One year of experience in case management, utilization review, or equivalent
- Regulated healthcare professional
- Member in good standing of a professional college
- Minimum one to two years of leadership experience
Working Conditions
This position requires working in a fast-paced healthcare environment, with frequent interactions with patients, families, and healthcare teams. The successful candidate will be able to work effectively in a team setting and adapt to changing priorities and deadlines.
