We are seeking a highly skilled and compassionate Registered Nurse to join our team at Home and Community Care Support Services. As a Registered Nurse, you will be responsible for providing high-quality patient care in a community setting, working collaboratively with patients, families, and healthcare professionals to achieve optimal health outcomes.
Key Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare professionals.
- Conduct home visits to assess patient needs and provide education on self-care and disease management.
- Collaborate with community resources and healthcare professionals to ensure seamless care coordination.
- Document patient information and progress in a timely and accurate manner.
- Bachelor of Science in Nursing (BScN) or equivalent.
- Current registration with the College of Nurses of Ontario (CNO).
- Minimum two years of experience in acute hospital or community nursing.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic proficiency in computerized information systems.
- Case management experience or recent related community experience.
- Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Ability to speak French or another second language.
