About the Role

We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a key member of our interprofessional care team, you will provide treatment and care management services to individuals with mental illness and concurrent disorders.

Key Responsibilities
  1. Coordinate care for clients by collaborating with the interprofessional care team, performing intake functions, conducting mental health assessments, and developing comprehensive treatment plans.
  2. Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing treatment modalities such as ADLs, wound care, and medication administration.
  3. Work from a strengths-based model, using motivational interviewing and engagement techniques to develop and implement individualized care plans.
  4. Establish and maintain therapeutic relationships with clients and their informal support network, providing support and follow-up planning, and promoting self-care and informed decision-making.
  5. Provide treatment services, including crisis intervention, individual counseling, and education, using a variety of theoretical models.
  6. Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team to ensure a coordinated approach to treatment.
  7. Consult and collaborate with members of the interprofessional care team, including MH&SUS Physicians, Primary Care Providers, and other healthcare professionals/providers.
  8. Monitor Extended Leave as required under the Mental Health Act, and aid client access to services such as medical appointments, hospitals, and social programs.
  9. Document assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
  10. Maintain a safe environment for clients, visitors, and employees by adhering to established safety and emergency practices and procedures.
Requirements

To be successful in this role, you will require:

  • Graduation from an approved School of Nursing or Psychiatric Nursing.
  • Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid Class 5 BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
