We are seeking a highly skilled and compassionate Psychiatric Liaison Nurse to join our team at Fraser Health. As a key member of our interprofessional care team, you will play a vital role in providing person-centered and recovery-oriented care to patients with psychiatric and concurrent disorders.
Key Responsibilities- Perform intake assessments and offer crisis intervention to patients in the Emergency Department
- Provide input into the care of psychiatric patients awaiting transfer for further assessment or inpatient admission
- Collaborate with Emergency Department nurses and other healthcare professionals to ensure effective patient flow and care coordination
- Develop and implement care plans that address psychiatric needs, with final dispositions authorized by Emergency Room physicians or consulting psychiatrists
- Work closely with interprofessional community mental health teams, agencies, and other community service providers to ensure seamless care transitions
- Provide short-term follow-up treatment and case management services to patients at the Mental Health Centre
- Graduation from an approved school of Nursing or Psychiatric Nursing
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives
- One year recent related experience in acute or community mental health and substance use setting
- Strong assessment and intervention skills, with ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills, with ability to work collaboratively with patients, families, and healthcare teams
