Position Overview
The Occupational Therapist plays a crucial role within the interprofessional Dual Diagnosis Consultation Outreach Team (DDCOT) dedicated to serving Southeastern Ontario. This team specializes in providing assessment, consultation, and short-term therapeutic interventions for individuals aged 16 and older who present with a dual diagnosis, which includes both an intellectual disability and a diagnosed or suspected mental health condition, including behavioral disorders.
In a collaborative care model, the team partners with community organizations and family practitioners to mitigate the effects of mental health challenges and enhance the quality of life for adults with dual diagnoses and their support networks.
Qualifications and Experience- Bachelor's degree in Occupational Therapy, with a minimum of five years of relevant experience; a Master's Degree in Occupational Therapy is preferred.
- Current, unrestricted registration with the College of Occupational Therapists of Ontario.
- At least three years of experience in dual diagnosis or psychiatric settings is preferred.
- Clinical experience with individuals who have a dual diagnosis is advantageous.
- Experience in diverse community environments, including home-based settings.
- Demonstrated ability to work collaboratively with families and community organizations.
- Experience in community development is preferred.
- Experience in providing case management services.
- Conducts thorough assessments, consultations, and treatments for a clinical caseload, including follow-up and discharge planning.
- Reviews and updates treatment and care plans in collaboration with clients, caregivers, and healthcare providers.
- Identifies additional support and educational needs for clients and their caregivers.
- Participates in case conferences related to client care as needed.
- Evaluates risk factors and intervenes appropriately; advises on equipment and adaptations for various care settings.
- Monitors therapeutic interventions and assists in tracking pharmacological treatments for effectiveness and side effects.
- Addresses behavioral challenges using specialized techniques such as sensory integration.
- Facilitates communication needs and adapts methods to effectively engage with individuals with varying communication abilities.
- Reports significant changes in client status to relevant members of the healthcare team, including family physicians.
- Documents client progress and responses to therapeutic interventions in their case records.
