About the Opportunity

Reporting to the Manager of Primary Health Care, the Clinical Pharmacist will collaborate with a multidisciplinary team of healthcare providers within family practice and chronic disease management teams. The focus is on delivering patient-centered care that prioritizes safe, effective, and appropriate medication therapy.

Key responsibilities include:

  • Working alongside interprofessional teams to optimize drug therapy for patients.
  • Identifying and resolving potential drug-related issues.
  • Completing medication reconciliations to ensure accurate patient records.
  • Participating in interprofessional medical rounds to contribute to comprehensive patient care.
  • Providing clear and concise patient education on medications and their effects.
  • Offering therapeutic recommendations to the healthcare team based on clinical expertise.

The Clinical Pharmacist also plays a vital role in ensuring safe and effective drug distribution systems for both inpatient and outpatient settings. This contributes to the overall success of Nova Scotia Health Pharmacy Services in achieving its goals and objectives.

About You

We are seeking candidates who possess:

  • A degree from an accredited university program in Pharmacy.
  • Completion of a Pharmacy Residency Program is preferred.
  • Current licensure to practice Pharmacy in Nova Scotia.
  • Minimum two years of experience in clinical practice within a hospital pharmacy setting (preferred).
  • A strong understanding of pharmacology and therapeutics.
  • Excellent written and verbal communication skills.
  • The ability to provide accurate and timely recommendations regarding safe and cost-effective medication use.
  • Fluency in other languages is an asset, with French being particularly valuable.

Hours of Work

This is a permanent full-time position; 75 hours bi-weekly.

Compensation and Benefits

$53.24 Hourly
