Clinical Social Worker

Job Summary:
We are seeking a highly skilled and compassionate Primary & Community Care Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive social work and psychosocial services to clients and their families, working collaboratively with healthcare professionals to ensure seamless care delivery.
Key Responsibilities:
- Conduct comprehensive social work and psychosocial assessments of clients and their families, using a variety of assessment tools to identify needs and develop treatment plans.
- Provide therapeutic services, including counseling and support, to clients and their families, exploring psychosocial responses to identify problems and facilitate change.
- Collaborate with the interdisciplinary team to coordinate services, manage cases, and facilitate meetings to ensure effective care delivery.
- Participate in client care planning, assessing and determining care plans and types of services required, and evaluating client responses to interventions.
- Support a seamless approach to care and service delivery across the healthcare system, participating in admission and discharge processes, and arranging meetings to develop and coordinate comprehensive care plans and community services.
- Partner with community services, health, and social agencies to assess and manage individuals at risk, making referrals to appropriate resources and exchanging relevant information.
- Facilitate timely exchange and provision of information regarding client care between the acute care facility and the community service, ensuring individualized discharge planning and client care needs are met.
- Act as a clinical resource for clients and their families, providing information regarding community resources and access to these resources, and facilitating referrals where appropriate.
- Maintain patient records, documenting assessments, charting information, and preparing progress notes and reports in accordance with established standards, policies, procedures, and current regulatory requirements.
- Develop, coordinate, and deliver in-services, presentations, psycho-educational, and support groups, and/or training, collaborating with members of the interdisciplinary team to determine relevant content and plan and coordinate seminars.
- Participate on local committees and planning bodies to identify opportunities for change, assess available resources, analyze outcomes, and identify areas to enhance patient care services and reduce service gaps.
- Initiate, develop, and carry out research activities and program planning and development relevant to the needs of the patient, families, hospital, and community, as assigned.
Qualifications:
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting, or an equivalent combination of social work education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
Skills and Abilities:
- Comprehensive knowledge and understanding of counseling and clinical practice models as it relates to individuals, families, and groups.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessments.
- Comprehensive counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patients, families, and their support networks.
- Comprehensive knowledge of pertinent/relevant healthcare and privacy legislations.
- Working knowledge of First Nations, Metis, and Inuit health, culture, and traditions.
- Ability to educate and provide consultation to patients, families, and healthcare professionals.
- Ability to effectively work independently and with others, including working effectively in a multicultural environment, as part of an interdisciplinary team.
- Demonstrated ability to communicate effectively both verbally and in writing.
- Demonstrated ability to organize and prioritize workload in a complex environment.
- Ability to operate related equipment, including applicable software applications.
- Physical ability to carry out the duties of the position.
