Clinical Social Worker

As a Clinical Social Worker with Fraser Health, you will play a vital role in providing psychosocial assessments and developing treatment plans for assigned clients. Your expertise will help identify patient and family issues, and you will work collaboratively with the interdisciplinary team to set goals and objectives for client care.
Key Responsibilities
Formulate psychosocial assessments and treatment plans by reviewing referrals and gathering information about clients and their significant others.
Provide clinical and supportive counseling services to patients and families regarding identified problems, adjustment to illness, and personal concerns.
Develop and implement crisis intervention and resolution plans, as well as provide support services as needed.
Collaborate with clients, families, and staff to provide consultative and direct services regarding community and rehabilitation resources.
Participate in quality improvement activities by providing input into the development, implementation, and evaluation of departmental and program goals, objectives, policies, and procedures.
Qualifications
Master's degree in Social Work from an approved school of Social Work, plus two years' recent, related clinical experience.
Current full registration with the BC College of Social Workers.
Comprehensive knowledge of systems theory and ability to apply this theory to practice, Social Work assessment and intervention.
Fully developed and information-seeking skills that promote communication and lead to a co-operative approach to problem solving within an interdisciplinary setting.
Ability to establish rapport with clients and family members, and communicate effectively both verbally and in writing.
Ability to organize and prioritize work, and operate related equipment.
