Job Summary
We are seeking a skilled Clinical Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive psychosocial assessments, identify patient and family issues, and develop treatment plans to address their needs.
Key Responsibilities
* Conduct thorough psychosocial assessments to identify patient and family issues
* Develop and implement treatment plans to address patient and family needs
* Provide clinical and supportive counseling services to patients and families
* Collaborate with the interdisciplinary team to develop comprehensive care plans
* Provide education and support to patients and families regarding treatment options and lifestyle changes
Qualifications
* Master's degree in Social Work from an approved school of Social Work
* Current full registration with the BC College of Social Workers
* Comprehensive knowledge of systems theory and ability to apply it to practice
* Excellent communication and interpersonal skills
