Clinical Social Worker, Primary Care Specialist

Job Summary:
The Clinical Social Worker, Primary Care is a key member of our interdisciplinary team, working in Urgent and Primary Care Centres, Primary Care Clinics, or Primary Care Networks. This role involves conducting comprehensive social work and psychosocial assessments, providing treatment services, and collaborating with community agencies and healthcare professionals to ensure seamless care delivery.
Key Responsibilities:- Conduct comprehensive social work and psychosocial assessments of clients and their families, using a variety of assessment tools to identify problems and develop treatment plans.
- Provide clinical support to the interdisciplinary team, including physicians and nurse practitioners, in the development of care plans and services.
- Deliver and evaluate clinical services and outcomes, ensuring timely care to residents and continuous longitudinal primary care to unattached vulnerable residents.
- Participate on local committees and planning bodies to identify areas for improvement in service delivery and enhance patient care services.
- Maintain patient records, documenting assessments, charting information, and preparing progress notes and reports in accordance with established standards and policies.
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting.
- Valid B.C. Driver's license and access to a vehicle with appropriate business coverage.
- Comprehensive knowledge and understanding of counseling and clinical practice models.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessments.
- Comprehensive counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief.
- Ability to educate and provide consultation to patients, families, and healthcare professionals.
- Ability to work effectively in a multicultural environment, as part of an interdisciplinary team.
