Clinical Social Worker

About the Role:
We are seeking a highly skilled and compassionate Clinical Social Worker to join our In-Patient Psychiatry team at Fraser Health. As a key member of our interdisciplinary team, you will provide clinical social work counselling services to patients and families, conducting comprehensive assessments and developing care plans to ensure optimal continuity of care.
Key Responsibilities:
- Conduct comprehensive social work and psychosocial assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
- Develop and implement individualized care plans, coordinating with healthcare professionals and community services to ensure seamless transitions.
- Provide education and support to patients, families, and healthcare professionals on mental health issues, recovery, and community resources.
- Collaborate with the interdisciplinary team to develop and deliver in-services, presentations, and psycho-educational groups.
- Participate in research activities and program planning to improve patient care and services.
Requirements:
- Master's degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers.
- Two years' recent, related clinical experience in a mental health treatment setting.
