Job Title: HEART@Home Navigator

Join Humber River Health as a HEART@Home Navigator and play a vital role in coordinating seamless transitions in care for medically complex and/or frail elderly patients.

About the Role:

This Temporary Full-Time position is part of our Integrated Health Systems & Partnerships team, working closely with social workers, managers, and community partners to ensure the best possible outcomes for our patients.

Key Responsibilities:
  • Collaborate with healthcare teams to identify patients requiring HEART@Home services
  • Coordinate referrals, review patient information, and develop care plans
  • Lead transitional care conferences and coordinate care delivery
  • Develop and maintain relationships with community partners and healthcare teams
  • Monitor patient outcomes and implement quality improvement strategies
Requirements:
  • Successful completion of Nursing, MSW, or related discipline
  • Licensed healthcare professional with current registration and membership in good standing
  • 3 years of experience in care coordination, navigation, or advocacy
  • Project work experience in a healthcare environment (preferred)
