Job Title: Client Care Navigator

At Vision Loss Rehabilitation Canada, we are seeking a highly skilled and compassionate Client Care Navigator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities:
  • Assess client needs and develop personalized care plans to achieve optimal outcomes.
  • Coordinate and schedule appointments with primary and specialized service providers.
  • Monitor client satisfaction and take proactive steps to address any concerns.
  • Identify opportunities for new or emerging care options to meet changing client needs.
  • Ensure accurate and timely documentation of client health records.
Requirements:
  • 3-5 years of client service experience in the health and/or social service sectors.
  • Experience working directly with individuals living with vision loss.
  • Knowledge of available community services and clinical, social, and residential care options.
  • Strong communication and interpersonal skills, including shared decision-making and facilitation.
  • Ability to prioritize workload and manage competing tasks.
Preferred Qualifications:
  • Registered health professional designation.
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline.
  • 3 or more years of experience working with individuals with vision loss.
