Job Title: Renal Social Worker

We are seeking a skilled and compassionate Renal Social Worker to join our team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide essential psychosocial support to patients and their families navigating the complexities of renal care.

Key Responsibilities:
  • Conduct thorough psychosocial assessments to identify client needs and develop personalized treatment plans.
  • Provide counseling services to clients and their families, addressing concerns related to illness, lifestyle changes, and treatment objectives.
  • Offer education and support to individuals, couples, and groups, empowering them to manage the psychosocial implications of renal treatment.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordinate access to community and rehabilitation resources.
  • Participate in quality improvement initiatives, contributing to the development and evaluation of departmental goals and policies.
Requirements:
  • Bachelor's degree in Social Work from an approved institution.
  • Two years of recent experience in clinical social work, preferably in an acute care or community health setting.
  • Current registration with the BC College of Social Workers.
  • Valid BC Driver's License and access to a personal vehicle for business purposes.
