Position Overview
Vancouver Coastal Health is on the lookout for a Patient Care Coordinator (Acute & Ambulatory Care) to enhance their team in North Vancouver, BC. This role is ideal for a Registered Nurse eager to contribute positively to patient outcomes.
- Oversee patient care operations and offer leadership and support to clinical staff
- Facilitate smooth patient transitions and accessibility within various units/programs
- Serve as a clinical expert, mentor staff, and guide the interdisciplinary team
- Assist with administrative responsibilities, including staffing and scheduling, while promoting educational initiatives and research
- Work collaboratively with team members to improve patient-focused care
Required Qualifications
Education & Experience
- Valid Registered Nurse certification in British Columbia
- Post-basic specialty nursing certification as applicable
- A minimum of three years of relevant clinical experience
- At least one year of supervisory or administrative experience
- A valid BC Driver's License may be necessary
Essential Knowledge, Skills & Abilities
- Understanding of evidence-based nursing practices
- Capability to provide leadership, oversight, and expert consultation
- Excellent communication and collaborative skills
- Adept at prioritizing tasks and resolving issues
- Proficient in computer use and physically capable of fulfilling job responsibilities
