Position Overview

Are you passionate about enhancing the quality of life for individuals in need? Personal Support Workers (PSWs) play a crucial role in the healthcare team, providing essential support and encouragement to foster independence among patients in both home and community care environments. In this position, you will assist with daily living activities under the guidance of our Personal Support Manager.

We are seeking Personal Support Workers (PSWs) to deliver compassionate and family-centered care to patients in the Grand Bend & Forest region. Are you the right fit for this role?

Responsibilities of the Role

  • Assisting patients with daily living tasks, including personal care routines, household chores, respite care, medication management, and special functions.
  • Ensuring a safe environment by applying proper body mechanics, safe transfer techniques, and home safety practices.
  • Offering cognitive, social, and emotional support to enhance the well-being of patients.
  • Preparing nutritious, diverse, and budget-friendly meals and snacks, including accommodating special dietary needs as outlined in the care plan, while adhering to proper food preparation, storage, and hygiene standards.

Qualifications

  • A strong commitment to making a positive impact in the lives of patients.
  • Excellent interpersonal and communication skills.
  • Ability to work independently as well as collaboratively within a team.
  • Flexibility to work non-traditional hours, including early mornings, evenings, and alternate weekends.
  • A valid driver's license and access to a vehicle.
  • Certification as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program.
  • To ensure patient safety, a current vulnerable sector check screening and a clear background check are required.

Beyond home-based health care, CarePartners actively engages with the community through clinics, transitional care units, and provides support in retirement homes and shared care environments. Our Community Nursing Services outreach program has been delivering staff-led medical care and clinics in areas with limited healthcare access since 2009.
