Position Overview

The Client Care Coordinator plays a pivotal role within BC Mental Health & Substance Use Services, contributing to the holistic care of clients in a supportive environment.

Key Responsibilities

  • Lead and manage client care initiatives, fostering innovative strategies for service delivery. Collaborate with a multidisciplinary team to establish tailored care plans that address the comprehensive needs of clients, including their physical, mental, and emotional well-being.
  • Support staff in implementing care standards and organizational policies, ensuring compliance with established protocols. Engage with clients and team members to minimize deviations from care pathways, facilitating service evaluations to meet program objectives.
  • Participate in initial client assessments and interdisciplinary meetings, providing insights on care effectiveness and administrative decisions. Conduct thorough evaluations of client needs, performing risk assessments and prioritizing care requirements.
  • Monitor client progress and provide guidance to healthcare team members. Encourage interdisciplinary collaboration for treatment strategies, crisis management, and resource coordination, while offering clinical insights on complex cases.
  • Build and maintain therapeutic relationships with clients, promoting their involvement in care planning and self-management. Educate clients on managing their health conditions and empower them to enhance their quality of life.

Qualifications

  • Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
  • Completion of a post-basic specialty nursing program and a minimum of three years of relevant experience in the clinical field, including one year of supervisory experience.
  • Possession of a valid driver's license and access to a vehicle for local travel may be necessary.
  • Successful completion of Occupational First Aid II or equivalent.

Skills and Expertise

  • Strong understanding of primary care practices, mental health disorders, and substance use treatment.
  • Proficient in evidence-based nursing practices, including pharmacology and therapeutic techniques.
  • Ability to lead and supervise effectively, conducting comprehensive clinical assessments and formulating treatment plans.
  • Excellent communication skills, both verbal and written, with the ability to collaborate with various stakeholders.
  • Commitment to understanding and addressing the needs of Indigenous populations and promoting reconciliation efforts.
