About the Role

We are seeking a highly skilled Disability Case Manager to join our team at Alight. As a Disability Case Manager, you will play a critical role in assisting employees who are off work or at risk of being off work due to health reasons.

Key Responsibilities

  • Conduct thorough case assessments, gathering information from employees and their direct managers to understand the situation and identify barriers to a successful return-to-work.
  • Develop and implement strategies to address identified barriers and support return-to-work or an alternative plan.
  • Communicate proactively with the employer's HR representative to ensure seamless transition and coordination.
  • Manage the transition to Long-Term Disability (LTD) and provide medical and summary case management information to the LTD insurer.
  • Attend team meetings and participate in other projects as assigned.

Requirements

  • Minimum 5 years of Disability Case Management experience (STD, LTD, RTW) or equivalent combination of experience and specialized education.
  • University degree in a related field, with additional coursework in occupational health, rehabilitation, legislation related to disability management, and helping relationship skills.
  • Strong interpersonal and helping skills, with superior assessment and problem-solving skills.
  • French/English language skills are an asset.
  • Demonstrated ability to manage complex tasks under tight deadlines.
