Job Summary:

The Screening & Outreach Coordinator will serve as the primary point of contact for all referrals to the LEAD case management program. This role requires effective communication and collaboration with various partners involved in referring and receiving referrals for LEAD.

Key Responsibilities:
  • Provide an active point of contact for all referrals to LEAD.
  • Conduct initial screenings with new referrals, assessing and addressing emergent client needs.
  • Engage in street outreach to connect with individuals referred to LEAD.
  • Coordinate the assignment of new referrals to a case manager.
  • Develop and maintain positive relationships with LEAD partners and other service providers.
  • Document all activities according to agency and contract requirements.
Requirements:
  • High school diploma or equivalent required; undergraduate degree preferred.
  • Relevant experience may substitute for degree.
Preferred Qualifications:
  • Demonstrated ability to provide street-based outreach and engagement services.
  • Understanding of substance use disorders and harm reduction strategies.
  • Strong interpersonal and communication skills.
  • Excellent organizational skills and ability to prioritize workload.
Working Conditions:

The employee may be exposed to illicit drug residues and fumes. The employee must occasionally lift and/or move up to 30 pounds.
