Cancer Care Transition Specialist

Hamilton Health Sciences is seeking a skilled professional to fill the role of School and Work Transitions Program Counsellor. This position will provide one-to-one vocational and academic counselling to eligible clients/patients, helping them navigate the challenges of education and employment after cancer treatment.
Key Responsibilities- Provide individualized vocational and academic counselling to clients/patients, identifying education and employment barriers and developing strategies to overcome them.
- Collaborate with healthcare providers, educators, and counsellors to ensure comprehensive support for clients/patients.
- Attend school and work meetings, provide educational and informational program presentations, and promote the POGO School and Work Transitions program initiatives.
- Liaise with After Care clinic staff, establish links with local educational institutions and community agencies, and participate in program training initiatives.
- Track service delivery volumes, provide updates for the POGO School and Work Transitions Program webpage, and participate in program evaluation.
- Minimum undergraduate degree in Secondary/Post Secondary Education, Psychology, Disability Studies, or related areas.
- Minimum 3 years' experience in career and/or vocation counselling in a healthcare or education setting.
- Experience providing academic advisement to adolescents and young adults, coaching and supporting others with job preparation, and providing equitable and inclusive care to diverse populations.
- Demonstrated understanding and commitment to equity, diversity, and inclusion, with evidence of education and learning in this area.
- Valid driver's license and access to vehicle, knowledge of employment and education barriers for diverse populations, and knowledge of disclosure and workplace accommodations practices.
