Job Summary

The Clinical Care Specialist will provide evidence-informed and integrated care to registered outpatients of the Schizophrenia Outpatient Clinic (SOC) and Transitional Outpatient Program for the Schizophrenia Service (TOPSS) at St. Joseph's Healthcare Hamilton.

Key Responsibilities

  • Provide autonomous healthcare services to patients with schizophrenia and related psychotic disorders in a community-based setting.
  • Collaborate with physicians, other members of the interdisciplinary team, and clients/families to deliver comprehensive care.
  • Develop and implement individualized care plans to support patients in regaining and managing their health.
  • Participate in quality improvement initiatives to ensure best practices in mental health care.

Requirements

  • Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as NP - Primary Health Care.
  • Master's Degree in Nursing required.
  • Proof of successful completion of controlled substances education required.
  • Demonstrated experience with psychiatric assessment and providing pharmacologic and non-pharmacologic interventions.
  • At least three years recent demonstrated experience as a Nurse Practitioner with experience in community-based settings servicing individuals diagnosed with schizophrenia and related psychotic disorders.

Preferred Qualifications

  • Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution.
  • CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) preferred.
  • Demonstrated knowledge of legislative and ethical responsibilities relating to scope of practice and healthcare.
  • Excellent interpersonal skills and ability to maintain effective collegial relationships and teamwork.
