As a key member of our Hospitals & Communities Integrated Services (HCIS) or Tertiary Mental Health Services team, you will play a vital role in providing timely, responsive, and evidence-based treatment and clinical supports to individuals with serious severe and persistent mental health problems, and/or problematic substance use and/or developmental disability issues.
•Collaborate with a multi-disciplinary care team to ensure comprehensive care and support for clients.
•Provide assessment, treatment, case management, community support, and urgent response to clients who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life.
•Utilize specialized treatment modalities/therapies for individuals, groups of clients, and/or their families, and work closely with Intensive Case Management teams to provide team-based care coordination.
•Fulfil a vital function for consumers by working with them to realize personal recovery goals, build a trusting and productive relationship, and provide the support and resources needed to achieve goals, stabilize life, and improve quality of life.
This is a relief part-time (0.50 FTE) position until February 28, 2025 or the return of the incumbent.
Shifts of work are days 08:00 to 16:00, working alternating Monday and Tuesday, and Monday to Wednesday.
Effective April 1, 2024, the hourly wage will be $55.91 per hour.
About this Location/Unit:
Intensive case management services are a key part of the continuum of mental health services and supports for people with serious mental illness. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the consumer. The direct involvement of the consumer and the development of a caring, supportive relationship between the case manager and the consumer are integral components of the intensive case management process. Intensive case management is responsive to consumers' multiple and changing needs, and plays a pivotal role in coordinating required services from across the mental health system as well as other service systems (i.e., criminal justice, developmental services, and addictions).
