About the Job:

The Clinical Care Coordinator will function as a member of the larger Hospitals & Communities Integrated Services (HCIS) or Tertiary Mental Health Services multi-disciplinary care team.

Key responsibilities include ensuring timely, responsive, evidence-based treatment and clinical supports across the continuum of available services for individuals with serious severe and persistent mental health problems, and/or problematic substance use and/or development disability issues who may or may not be in crisis.

The Clinical Care Coordinator will provide assessment, treatment, case management, community support, and as required, urgent response to clients who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life.

The role involves functioning as a primary clinician utilizing a variety of specialized treatment modalities/therapies for individuals, groups of clients and/or their families.

Intensive Case Management teams help clients with severe mental health and/or substance use concerns through team-based care coordination. Services may include Substance Use Counselling & Treatment Services plus housing assistance, primary care & outreach.

The Clinical Care Coordinator will build a trusting and productive relationship with the consumer and provide the support and resources that the consumer needs to achieve goals, stabilize their life and improve their quality of life.

This is a relief part-time (0.50 FTE) position until further notice.

Shifts of work are days 08:00 to 16:00 working alternating Monday and Tuesday, and Monday to Wednesday.

The hourly wage will be $41.42 - $55.91 per hour.

About this location/unit:

Intensive case management services are a key part of the continuum of mental health services and supports for people with serious mental illness. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support as needed by the consumer.

The direct involvement of the consumer and the development of a caring, supportive relationship between the case manager and the consumer are integral components of the intensive case management process.

Intensive case management is responsive to consumers' multiple and changing needs, and plays a pivotal role in coordinating required services from across the mental health system as well as other service systems (i.e., criminal justice, developmental services, and addictions).
