Job Summary

We are seeking a highly skilled Social Worker to join our team at Southlake Regional Health Centre. As a Social Worker, you will play a vital role in delivering high-quality healthcare services to our patients and their families.

Key Responsibilities
  • Provide patient-centered care and support to individuals with cardiovascular disease and their families.
  • Develop and implement individualized care plans to promote optimal health outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and families on chronic disease management and self-care strategies.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Masters of Social Work from an accredited university.
  • Previous experience relating to patients with cardiovascular disease an asset.
  • Previous experience in Chronic Disease Management an asset.
  • Previous experience in Cardiac Rehabilitation setting.
  • Membership in the Ontario College of Social Workers and Social Service Workers.
