About the Role:We are seeking a highly skilled and compassionate Social Worker II to join our interdisciplinary team. As a Social Worker II, you will work collaboratively with healthcare professionals to provide evidence-based social work services to individuals with severe and complex concurrent substance use and mental health disorders.
Key Responsibilities:- Provide biopsychosocial assessments, consultations, and treatment plans for clients with complex needs.
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
- Advocate for clients and their families to access necessary resources and services.
- Participate in crisis intervention and risk assessment, and develop crisis plans as needed.
- Engage clients and their families in clinical interventions and risk reduction strategies.
- Assess clients' social determinants of health and provide support as needed.
- Document client information accurately and in a timely manner.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Master's degree in Social Work from an accredited institution.
- Registration with the Ontario College of Social Workers and Social Service Workers.
- Experience working with individuals with severe and complex substance use and mental health disorders.
- Training and experience in individual, family, and group work.
- Knowledge of harm reduction interventions and safer injection practices.
- Ability to work effectively in an interdisciplinary team environment.
- Excellent communication and interpersonal skills.
The Royal Mental Health Centre offers a competitive salary range of $44.67 to $57.90 per hour, as well as opportunities for professional growth and development. We are committed to creating a diverse and inclusive work environment that values the contributions of all employees.
