Care Support Specialist

AVI Health and Community Services is a leading provider of HIV, hepatitis C, and harm reduction services in Vancouver Island. We are seeking a highly skilled and compassionate Care Support Specialist to join our team on a temporary part-time basis.
Key Responsibilities- Provide direct services to individuals living with HIV and/or hepatitis C, utilizing a harm reduction model to deliver care and support.
- Engage with clients in a collaborative and person-centered approach, using brief assessment, crisis response, and counseling skills to address immediate needs.
- Advocate on behalf of clients with various government agencies and health services to ensure access to maximum support and information.
- Work with clients to address issues related to housing, medical, legal, and other support agencies.
- Facilitate bi-monthly group sessions for priority populations.
- Attend appointments with clients in the community and provide harm reduction supplies and education.
- Provide testing information, education, and referrals to clients both on-site and during outreach activities.
- Offer onsite supervision and support to clients.
- Provide support and referrals to individuals who have experienced overdose, including Opiate Agonist Treatment (OAT) and other resources.
- Participate in regional HIV and hepatitis C community initiatives.
- Diploma in health or social services (Social Work, Psychology, Mental Health & Addictions Support Worker, etc.).
- A minimum of two years' relevant work experience in direct service delivery and advocacy.
- Valid Class 5 BC Driver's License and access to a vehicle for client transportation.
- Able to respectfully engage and support Indigenous relationships, with a trauma-informed and decolonial lens.
- Demonstrated ability to establish rapport, provide support, and advocate for a diverse range of clients.
- Commitment to social justice, equity, anti-racism, and decolonization.
- Understanding of psycho-social issues affecting individuals living with HIV and/or hepatitis C.
This part-time position is based at the AVI Courtenay Office and requires occasional community work. The successful candidate will be part of a supportive and dedicated team committed to promoting the health, dignity, and well-being of individuals affected by HIV, HCV, and substance use.
