About the Role

We are seeking a highly skilled and compassionate Social Worker to join our In-Patient Psychiatry team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide comprehensive social work services to patients and their families, supporting their mental health and well-being.

Key Responsibilities
  • Conduct comprehensive social work and psycho-social assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
  • Develop and coordinate care plans, working closely with patients, families, and healthcare professionals to ensure optimal continuity of care.
  • Provide education, support, and consultation to patients, families, and healthcare professionals on issues related to mental health, addiction, and recovery.
  • Collaborate with community health and social agency personnel to ensure effective referrals and resource utilization.
  • Maintain accurate and comprehensive patient records, documenting assessments, progress notes, and reports as required.
Requirements
  • Master's Degree in Social Work from an accredited school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • At least one year's recent, related experience in a mental health treatment setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multicultural environment.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
